Please fill out the following form. This document releases the UNBC TimberWhales Underwater Hockey Team from any liable injury or illness obtained while playing underwater hockey with the team. It also allows us to use your photo/video on our social media pages. You will not be allowed to play without signing this form. All this information will remain confidential and will not be released to anyone other than the executive team. Thank you!
